Common causes

• POST to /api/<table>/<id> (create only on the collection).
• PATCH or DELETE without an id (bulk updates and deletes are not supported).
• Anything other than POST to /mcp (stateless Streamable HTTP).

How to fix

Use POST /api/<table>, PATCH /api/<table>/<id> or DELETE /api/<table>/<id>.
